Capital Blue Cross’ care management program is a free, voluntary service available to eligible Medicare Advantage members. It is designed to help you manage your health, better understand your care, and feel supported along the way through personalized one-on-one support. A specially trained care manager will work with you, your family, caregiver, and your doctors to help you understand your options, coordinate your treatment, and maintain or enhance your quality of life. It’s confidential and available at no extra cost to you.

Our team is here to support you one-on-one, so you don’t have to figure everything out on your own. They take time to listen to what matters to you, help you understand your health and your care plan, and work with you (and, if you choose, your family and caregiver) to coordinate next steps. Whether you have a quick question or you’re managing several health needs, we can help you feel more comfortable, more informed, and more in control.
- Support may include registered nurses, social workers, behavioral health specialists, health navigators, and clinical support specialists from across the country.
- Many hold specialty certifications in areas such as behavioral health, peer support, health navigation, transplant and oncology.
- Their combined experience includes social work, critical care, medical/surgical, home health, hospice, long-term care, oncology, transplant, and dialysis.
