Capital Blue Cross’ care management program is a free, voluntary service available to eligible Medicare Advantage members. It is designed to help you manage your health, better understand your care, and feel supported along the way through personalized one-on-one support. A specially trained care manager will work with you, your family, caregiver, and your doctors to help you understand your options, coordinate your treatment, and maintain or enhance your quality of life. It’s confidential and available at no extra cost to you.

Our team is here to support you one-on-one, so you don’t have to figure everything out on your own. They take time to listen to what matters to you, help you understand your health and your care plan, and work with you (and, if you choose, your family and caregiver) to coordinate next steps. Whether you have a quick question or you’re managing several health needs, we can help you feel more comfortable, more informed, and more in control.

  • Support may include registered nurses, social workers, behavioral health specialists, health navigators, and clinical support specialists from across the country.
  • Many hold specialty certifications in areas such as behavioral health, peer support, health navigation, transplant and oncology.
  • Their combined experience includes social work, critical care, medical/surgical, home health, hospice, long-term care, oncology, transplant, and dialysis.

Through proactive outreach, Health Navigation connected a Medicare Advantage member preparing for hip replacement surgery to personalized physical, behavioral, and social support during a critical time. By coordinating peer support, nutrition coaching, weight management resources, and post-surgical follow-up, the navigator helped reduce barriers and build confidence in recovery. With guidance and encouragement, the member safely managed her recovery, engaged in wellness programs, and initiated conversations with her provider about long-term pain management. This integrated, compassionate approach empowered the member to feel supported, connected, and optimistic about her ongoing health and well-being.

The care management team assists with mental health and substance use disorders. We aim to improve the quality of care for members experiencing behavioral health issues by integrating treatment plans, facilitating access to resources, and promoting member engagement in their recovery processes.

When an older adult member experienced worsening depression and health challenges after hospitalization, our Behavioral Health Care Management team stepped in with compassionate, coordinated support. Through timely intervention, motivational interviewing, and peer support, the team helped the member access care at a specialized facility better suited to her needs. With updated treatment and medication, the member regained strength, resumed healthy eating, and reconnected with her community and faith. Today, she is home, doing well, and continues to thrive with ongoing care management and peer support by her side.
